He is survived by his brother Robert (Polly) Polacek; sister Marcella Roider; dear friend Geraldine Uhl; nieces; nephews; and cousins.

Frank is preceded in death by his parents Frank G. & Mary L. (nee Ditte) Polacek; 2 brothers; and 2 sisters.
